The program offers tailored psychological interventions with qualified clinicians such as counselling for individuals or in families or care groups.
Parramatta Mission has recently been commissioned by COORDINARE which is the South Eastern NSW Primary Health Networks to deliver this new program called "Continuing to be Me @ home" [C2bMe@home].
The program is designed to reinforce identity and purpose, increase connectedness, develop hope, reduce symptomatology, and optimise functioning so older persons can continue to live with dignity.
With the funding and support the program aims to collaborate with GPs and existing aged care services in the region to tailor a program that is suited to the needs of the older persons and the community.
The program is delivered straight into people's homes and is available for people aged 65 years and over living in the community or Aboriginal and Torres Strait Islander people 50 years and over.
